Pharmacy support for community health services - a toolkit
This document has been written by London, Eastern and South East Pharmacy Services with the Primary and Community Care Pharmacy Network (PCCPN. PCCPN is the UK wide special interest
group for Pharmacy staff who support community health services (CHS). The purpose of the document is to alert commissioners & providers of CHS to the Pharmacy support needed to assure the safe use of medicines to meet 'Standards for better health'. The toolkit is relevant to any CHS where medicines are prescribed or administered to patient or where patients are supported to take or use their own medicines.
