When a patient changes home address and still requires oxygen the supplier should be informed as soon as possible. If the patient remains in the current suppliers area, there will need to be a change of delivery address.
However, where the patients moves outside of the area, and where a different supplier operates, it will be necessary for a new HOOF to be completed.
