The annual National Heart Failure Audit report highlights that fewer than 4% of patients with heart failure were referred to palliative care in 2010/2011. Referrals rates increased slightly to 6% for those readmitted to hospital.
The main purposes of the audit are to measure the quality of care and clinical outcomes, to enable comparisons between trusts or health boards, and to recommend changes and promote improvement where necessary.
