Data collection questionnaire for PNAs
Following seminars for LPCs and PCTs in March, PSNC has developed a template questionnaire that PCTs may wish to use when seeking information from their pharmacy contractors as part of the development of their first PNA. The questionnaire can be found on the PSNC website at http://www.psnc.org.uk/pages/forms.html.
If the template is to be used as produced then PCTs will need consent from PSNC to use the logo (contact steve.lutener@psnc.org.uk). Consent will generally be given so long as there are no questions added to the form. If you want to amend the form to meet local circumstances, please make sure that you remove the PSNC logo.
It is recommended that PCTs agree their template with their LPC before circulating it to contractors. PCTs should ensure that they only seek the information they require for their first PNA and may wish to pre-populate the form with the information they already hold therefore allowing contractors to validate PCT records.
