NHS Coventry have produced the following documents when dealing with voluntary GOS Contract Terminations.
Model form for GOS Contractors to advise PCTs and/or shared service agencies of voluntary termination of their GOS contract.

Guidance to care and nursing home staff about domiciliary sight tests
NHS Buckinghamshire has recently issued guidance to all staff of the local county council who work in care homes and day centres. This guidance emphasises the costs of domiciliary eye care and includes a warning about staff needing to be authorised before they can sign NHS forms on behalf of patients. A generic version of this guidance is reproduced here as an example of emerging practice which PCTs might wish to amend locally as required and then distribute to care and nursing home managers in the PCT’s own locality.
